Abstract: The article examines the economic and institutional mechanisms for enhancing the competitiveness of private healthcare services within Germany’s healthcare system. The German model is assessed not as an intermediate system between a free market and fully state-run healthcare, but as a combination of social insurance, regulated competition, and self-governance institutions. The study reveals the impact of competition among health insurance funds, risk adjustment through the Risk Structure Compensation Scheme (RSA), dual hospital financing, DRG-based reimbursement, patient choice, quality-based competition, and unified payment rules for private and public providers on the competitive position of the private sector. Based on the German experience, the study proposes the gradual introduction in Uzbekistan of risk-adjustment mechanisms, financing principles that prioritize performance over ownership, and elements of quality-based regulated competition.
